How to create your medical bill

Fields to fill in

Provider and patient
Enter the hospital, clinic or pharmacy details and the patient’s name. Add doctor, guardian and contact details where needed.
Dates and references
Use the invoice or cash-memo number and relevant bill dates. Hospital and clinic layouts include admission, discharge, room and insurance fields.
Medicines or services
List treatments in service rows. For pharmacy receipts, enter medicine names, quantities, rates and the supported batch, manufacturer and expiry fields.
Charges and remarks
Enter applicable tax, delivery charges and payment details for the chosen layout. Review signature options and any medical or return-policy notes.

Create and download in three steps

  1. Fill your chosen format. Choose a hospital or clinic bill for treatment charges, or a pharmacy format for purchased medicines and their batch or expiry details. Fill the patient and provider fields shown by that layout, using only the information needed for the receipt.
