What to fill in

Explain the stay and its charges separately
A stay period does not explain every line on the bill. Keep the unit and quantity of each charge visible, and reconcile it against the facility’s own supporting records.
Read the worked example
The example has INR 3,000 for room charges, INR 800 for a consultation and INR 1,200 for an investigation. Charges total INR 5,000; after a recorded INR 2,000 deposit, the remaining amount is INR 3,000.
When a patient needs the original record
Only the actual issuing facility can provide its original or corrected bill and relevant treatment records. This template cannot establish treatment, payment, eligibility or approval for an insurance or government claim.

Can this replace the hospital’s bill for a claim?
No. Obtain the actual bill and required records from the treating facility and follow the insurer or scheme’s current requirements.
Is this also a pharmacy invoice?
A pharmacy sale may need medicine, batch and other product details not covered by this stay example. Do not relabel it as a universal pharmacy or clinic form.
